I am humbled to share a significant milestone in pancreatic surgery: I recently performed the first-ever da Vinci single-port robotic distal pancreatectomy and splenectomy without an additional incision for pancreatic cancer in the United States, approved by both the Institutional Review Board and IDE from the FDA through a 2cm incision inside the belly button leaving no additional scar. This pioneering procedure not only achieved negative margins but also allowed for the thorough harvesting of all local lymphatics, a critical factor in ensuring comprehensive cancer treatment. I am grateful to report that the patient is doing exceptionally well, and it was a privilege to see him in the clinic several days post-operation. The images included illustrate the operating room moments and a snapshot of our follow-up clinic visit with my remarkable ARNP Dana Manzi and myself. This journey underscores the importance of innovation in surgical techniques and the profound impact they can have on patient outcomes. Thank you to everyone who has supported and believed in this work. Don't hesitate and #enucleate HD video on my Youtube channel Dr. Sanchez Ali Again a #HoLEP without editions for urologists who want to start enucleating, in this case, I show how at some point it is very easy to go from a very anatomical plane to performing a subtrigonal drilling and how to avoid continuing there and correcting the plane . As usual, the prostate capsule is very thin, it is common to make small or large capsular perforations, which in enculelation are maximized and in the end we can see that they are magnified by the flow of our nucleation equipment. As you can see at the beginning of the video, it is something of no major importance at the end of the surgery. #UroSoMe #urology Amer. Urol. There is hope. I got on the phone with an insurance medical director to advocate for a 34 year old elementary school teacher who needed breast reconstruction after her mastectomy. She had carefully planned her surgery during summer break so she could return to her students in the fall. But because Redbud Surgery Center remains out of network despite nearly two years of trying to become in network, she was facing thousands of dollars in additional costs. So I made the call. I explained that this wasn't about reimbursement. It wasn't about me. It was about a patient who deserved access to safe, high quality care without unnecessary financial barriers. And something remarkable happened. He listened. He understood why this mattered. He approved an exception for this patient. Then he acknowledged that Redbud Surgery Center being in network would be better for their members. For all the frustration, all the barriers, and all the moments when healthcare feels broken, I want you to know this: There are good people inside the system trying to do the right thing. Healthcare doesn't change through outrage alone. It changes when people are willing to listen, understand, and act. There is hope. And together, we are changing healthcare from the inside out. One patient. One doctor. One decision. One practice. One conversation at a time. My gratitude for the physician who listened and heard me is immense. Now I need your help. If you have Blue Cross Blue Shield of Texas insurance, please contact your employer's benefits manager or human resources department and tell them: "I want Redbud Surgery Center in network." Your employer has more influence than you may realize. Insurance companies listen when employers advocate for better access to care for their employees. If you live in Texas, please also contact your state representative, state senator, and the Governor's office and tell them: "I want Redbud Surgery Center in network for the people of Texas." If enough patients, employers, physicians, and Texans speak up, change can happen.

"Orthopedics Is By Far The Most Corrupt Form Of Medicine & The 2nd Most Corrupt Is Oncology." Gardiner Harris "Orthopedic Surgeons Will Choose Devices Based On Their Payouts & Bonuses...Not Patient Outcomes." "You Must Have An Advocate With You At The Hospital At All Times..." "Death by Doctor" is the 3rd leading cause of death in the America. A complete Iatrogenic death. Improperly Prescribed Drugs, Medical Malpractice & Surgical Errors Kill People Every Single Day. In 2016, There Were 250,000 Deaths From Medical Error. Current Medical Error Deaths Have Risen To 440,000 Per Year. That is more than 1,200 fatalities every single day in hospital settings. If you or a loved one is admitted to the hospital, it is imperative that someone is there at all times. Family should organize & stay in shifts around the clock in order to protect their health & very life. Being in the hospital makes that person in their most vulnerable position. Medicines, procedures & treatments can be ordered & given without the patient's consent. And when family leaves visiting hours, the patient is alone & defenseless against a 'sick care system' not looking out for the best interest of you or your loved one. Specifically For Orthopedics & The Dangers Therein: Orthopedic surgeons receive the highest compensation from the medical device industry compared to other specialties. Critics & government officials have expressed concerns that these payments influence medical decisions, leading to unnecessary procedures or the use of specific devices, thereby inflating healthcare costs & endangering patients. These payments can take various forms, including: 🔴Royalties & kick-backs for assisting in implant design. 🔴Speaker fees & all inclusive trips for promoting devices at medical conferences. 🔴Stock holdings in exchange for consulting work & choosing to use those implant & surgery devices exclusively. Federal laws, such as the Anti-Kickback Statute, aim to curb illegal financial inducements, but it continues to go on everyday within Orthopedics: 🔴DePuy Synthes Inc., a subsidiary of Johnson & Johnson, settled allegations that it violated the False Claims Act by providing kickbacks to an orthopedic surgeon in exchange for using their products. 🔴Device maker Life Spine and two of its executives admitted to paying consulting fees to incentivize surgeons to use their implants. 🔴Southeast Orthopedic Specialists agreed to pay a settlement for falsely billing for services not properly supervised by a physician. Patient safety is also a concern related to potential financial conflicts of interest. For example: 🔴A lawsuit was filed against a Florida device maker, Exactech. They offered "phony" consulting deals to surgeons who complained about defects in a knee implant, potentially silencing concerns that should have been reported to the FDA to protect the public. 🔴A Maryland spine surgeon is facing a lawsuit from patients claiming he implanted counterfeit hardware not approved by the FDA, after allegedly receiving substantial consulting fees from the device distributor. 👇DOJ Investigates Orthopedic Surgery Industry👇 👇Orthopedic Surgery Complications👇 👇Medical Error 3rd Leading Cause Of Death👇 Speaker: Gardiner Harris Video: WR

Hi everyone! Just rolled out access for NoSpoon Studios to the first 110 folks or so. Huge thanks to Karan and Amit at Luma - thanks to them, I have a few more people that had commented getting complimentary beta access soon as well :). These videos are created with a single agent that does everything from concept to completion (including editing the video together). Therer are longer options too, but for the sake of cost and this demo, you'll be working with 20s max generations. The entire agent was built to only run Ray2 as the video model. My stack is this custom agent of mine (using OAI) > Ray2 for the video > Replit for the app for now. In the meantime, for those with access: here's a video tutorial on how to use the site. I'm quite exhausted, but here's the gist: 1. Sign up (and remember your password lol). Beta testers got free credits, but these are a bit limited - rolling out to a few more peeps but have to budget and need to expand my current code (which means redeploying - so expect more of you to have 2 free credits and access tmr! I have to find out the max I can allot with budget). 2. Just hit "generate short video" without anything in the prompt box and the agent will start immediately with the logline. Be very patient, wait about 6-8 minutes, and it will continue to output all of the scenes and processing - it's very important you do not refresh or navigate away from that tab at this time. You'll see your final video generated and displayed at the bottom of your screen once complete, along with a watermark (paid users don't have a watermark). You can also access your loglines and videos in your history tab, too :). You must refresh the page only after your generation in order to generate a new video - otherwise you'll lose your generation (annoying I know). 3. You can also input your own custom prompt! Control the story this way. Single words work great even, like "thriller" or "spy thriller set in greece" or even "black and white film noir" etc etc. It's not as great with animated styles yet for consistency, just a heads up. 4. I'm exhausted, I'm sure there's a lot I'm missing. But welcome to the NoSpoon Studios soft beta! More soon! Feel free to share your generations if you are already in. I'll see you all tomorrow night :) <3 Thanks everyone! More soon.
