The NTSB released the security camera video showing the engine separating from the UPS MD-11 moments before it crashed. The docket also includes the inspection of the engines and in particular, engine two which showed a few performance blips prior to the crash but with recovery to commanded speeds. So, I don't believe there's evidence that debris were ingested into engine 2, instead the performance problems were related to the flames from the wing entering engine 2. Some of the footage appears to support this.

People are saying that this SRB nozzle failure in 2019 shows that there has been no progress. I disagree, yesterday's failure was more than just the nozzle and was far worse, so they've actually gone backwards in terms of test success.
