The mysterious concoction given to the referee here is probably pickle juice. Pickle juice relieves muscle cramps rapidly, often in under two minutes, not by rehydrating the body or restoring electrolytes, but through a reflex in the throat. The acetic acid in the vinegar triggers sensory receptors that send a signal to the nervous system, forcing overactive, cramping muscles to relax.

This was a real case in Germany of a surgeon who sustained a scalpel injury to his left had while resecting a malignant abdominal tumour known as a fibrous histiocytoma. 5 months later, the surgeon notices a tumour arising at the same wound site on his left hand. Molecular analyses confirms the implausible - that the surgeon’s tumour was also a histiocytoma. Histopathological and DNA analyses found this to be genetically identical and indisinguishable to the patient’s tumour. The surgeon's tumour was successfully removed via surgery. 2 years later, he was in good health without signs that the tumor had spread or was returning. With surgical injuries, we tend to worry about blood borne infections, but seeding and implantation of cancer cells is a rare but possible complication. 📽️: Zack D Films

*Loosely inspired by real life events* Hospital visits due to ingestion of wire bristles from BBQ cleaning brushes are an under-recognised cause of injury, with approximately 130 hospital visits in the US each year. Most injuries occur when detached wire bristles adhere to grilled food and are inadvertently swallowed. Because the bristles are thin (often 1–2 cm), sharp, and difficult to visualise on plain X-rays, diagnosis may be delayed. Clinical presentations reported in the literature include: 1. Oropharyngeal or tonsillar impaction, leading to throat pain or problems swallowing. 2. Hypopharyngeal or oesophageal penetration. 3. Migration into soft tissues of the neck with abscess formation. 4. GI perforation involving the oesophagus, stomach or small bowel. 5. Migration into the gallbladder has also been reported, caused by a wire fistulation through the stomach. Take home messages: be careful when cleaning the BBQ this summer! Consider this in a patient with unexplained throat pain after a BBQ. 📹: The Good Doctor
