During URSI (Ultra Rapid Sequence Intubation) use the videolaryngoscope... as a “spy around the corner” and see when the cords are opening 🎥 Important points: 1. insert the blade very gently, dont cause vomiting !) 2. this works well with high dose rocuronium, a bit more challenging with sux due to muscle depolarisation and twitching 3. use lubricating gel to lubricate the innercurve of the blade 4. place the patient in head-up position 5. have suction ready 👉intubation procedure can start after 30 seconds #speedygonzalez James DuCanto, M.D. Cliff Reid Dave Olvera carmine della vella INTUBATIEM Simon Carley Sam Ghali, M.D.show more

(🧵1/11) For the past year and a half, I've... been investigating OpenAI and Sam Altman for The New Yorker. With my coauthor Andrew Marantz, I reviewed never-before-disclosed internal memos, obtained 200+ pages of documents related to a close colleague, including extensive private notes, and interviewed more than 100 people. OpenAI was founded on the premise that A.I. could be the most dangerous invention in human history—and that its C.E.O. would need to be a person of uncommon integrity. We lay out the most detailed account yet of why Altman was ousted out by board members and executives who came to believe he lacked that integrity, and ask: were they right to allege that he couldn't be trusted? A thread on some of of our findings:show more

Case File - Trash Bag Killer Exhibit No -... AN2347 An image from the case file of The Trash Bag Killer In this image the victim lays dead on the bed having been strangled T.J., short for Tanner James, was an 18-year-old from the flat, fertile farmland near Galesburg, lllinois. He was born and raised on a family-owned grain and soybean farm that has been in the Jenkins family for over a century. His upbringing was a masterclass in hard work, responsibility, and the unspoken bond of family. His father, was a man of few words but immense integrity, taught him how to read the land and the weather. His mother, the heart of the home, ran the household and a small farm stand, instilling in him the values of kindness and community. He grew up as the middle child between two sisters. One of TJ’s interests was his prize possession a 1972 Chevy pickup that he was constantly doing work on. TJ needed a part and had been searching around for it for a while. He then spotted one online and made arranger to potentially buy it. The drive for the part was five hours and TJ was happy when he arrived and the seller said he had the part. Unbeknownst to TJ the seller was the serial killer known as the Trash Bag Killer and before he knew it TJ was incapacitated and strangled. The killer enjoyed his time with TJ’s body before dismembering it and disposing of it in trash bags. #serialkiller #aigeneratedshow more

A true test of ultimate patience and faith by... this TB gelding’s owners…this wonderful racehorse came to us in Nov 24 for 2nd opinion.Euthanasia had been recommended by another specialist for a very complicated stifle injury-a nasty joint infection combined w/a very painful bone contusion/cyst. He was crippled at the walk for months but an incredibly cooperative patient. At times, we really couldn’t see the light at the end of the tunnel but he made us keep trying. Intense medical treatment, physical therapy, and lots and lots of time. The owners kept saying “let’s give him another month” and then another, and another until he began to turn the corner! His X-rays filled in, he started using his leg, and exercise was gradually increased until….THIS! He is sound, happy, and finally going home this week🙌🏻🙌🏻🙌🏻 WOW #powerofpatienceshow more

The Ultimate Betrayal: Murd-er in the Name of a... Invite This is beyond disgusting. How can anyone be so heartless? A Young Hindu man was invited for 'Iftari' during Ramzan, thinking he was going for a meal, but it was actually a death trap. A massive mob of 15 people of Peaceful MU£lim Community was waiting for him, and they attacked him with zero mercy. The family is absolutely shattered, crying on camera because their son was literally hunted down. They used everything bricks, heavy stones, knives, and even guns to k!ll him. This wasn't just a fight; it was a brutal, planned execution. The most painful part? The family feels like nobody is listening to them, and they are begging for justice while the k!llers roam free. We cannot stay silent when someone is lured under the guise of hospitality and then slaughtered like this. This kind of cowardice needs the strictest punishment possible. Period.show more

Alex Collins was so special man. Amazing teammate and... a guy everyone was down to kick it with. This man invited me and wifey out one time in Baltimore and I will never forget it. We show up to the spot and AC is no where to be found. We are looking around for him for 20 min and finally I feel a little nudge on my shoulder. I turnaround and AC is walking behind me, drops his shades to his nose and signals with his eyes to follow him. It was so funny and smooth, we forgot he was 20 min late. AC told us he had to be discreet because he couldn’t have everyone knowing he was there. The rest of the night, we found out why. Everywhere AC went that night everyone knew him. Everyone was excited to see him, laughing when he came around and showing him love. You see everyone loved this man and his energy was contagious. To end the night, he hit this dance with all the fans watching him on the dance floor. WHAT A NIGHT and WHAT A GUY AC WAS. Shared more laughs than people know and I’m blessed to have been your teammate my brother. Rest In Peace.show more

The viral video of Emmerson Mnangagwa completely ignoring his... presidential advisor is not just a awkward political moment We warned Tungwarara. We shouted it from the rooftops that this man cares for no one, loves no one, and respects no one. But Tungamirai thought he was special. He thought he was different. He blinded himself to history and became the chief architect and loudest preacher of the "2030 Agenda." He threw himself into the trenches, moving heaven and earth to make Constitutional Amendment Bill Number 3 a reality, all to guarantee a term extension for a man who views people as nothing more than disposable tools. Now that the groundwork is laid, the mask is off. Tungamirai has been used, drained of his utility, and cast aside like a used condom. How could anyone be this politically blind? Look at the history! General Constantino Chiwenga did not just hand Mnangagwa the presidency on a silver platter; he literally saved his life. When Mnangagwa was poisoned at that Zanu-PF rally in Gwanda, it was Chiwenga who flew him out, saved his body from death, and engineered his path to State House. And what was the reward for that ultimate loyalty? Mnangagwa turned around and weaponized the entire 2030 Agenda for the sole purpose of blocking the very man who saved him from ever ascending to the presidency. There are even dark, persistent allegations that he has poisoned the very people who protected him. If a man can allegedly poison and politically destroy the General who saved his life and built his throne, who did a mere advisor think he was? This is the absolute truth of our current leadership. It is a system built on using patriots, milking them for power, and dumping them into the wilderness. Tungamirai wanted to play with fire, and now he is burning in the cold reality of who Mnangagwa truly isshow more

With a heavy heart I am sharing I lost... my baba last week. It was hard to come back home and not find him home. Being the first kid of my generation, I was loved by everyone in the family but my baba loved me the most. We shared a special bond. It’s unfortunate that I could not make it for his last rituals and something that I’ll regret forever but such is life 😞 He was a very jolly person, kids’ favourite and loved and respected by everyone in the town. He was 90+ and lived a good, fulfilling life. Humanity and kindness were deep rooted in him. May he rest in peace up there. Here’s a video of him from four years ago and just shows it was a treat to be around him.show more

Several months ago I read an article entitled "Fart... walks save lives" and ever since I've been a fan of a post-prandial stroll... It is so good for the digestion and general health; it gets the blood circulating, burns calories and let's you talk through the day with your other half....(farting is optional). So tonight, after a home-cooked meal, I persuaded james to come along the river for a 40 minute walk- and-talk. It was pitch black on the towpath and we saw only 3 other people, all wearing earpods . On the way back we heard, " help! Help!"... and at first thought it was one of the many rowing coaches you hear on The Thames - even after dark. But then I saw a bicycle on its side and shouted after the voice. 20 ft below us, in the water, was a bloke who had come off his bike and fallen headlong down the banking. He had flattened some of the bushes and trees but was still very difficult to see amongst the thick foliage. James scrambled down as far as safely possible to get a better look. The chap was still in his bike helmet, waist deep in the water and thick mud but unable to get any grip at all on the sheer brick wall. James got back up and found his very long bike lock which he then dangled down to try and pull him up - while I nervously said, " please don't get pulled in too.." (eventually James said, " yes I heard you the first 3 times..."). He was too far down to pull up but was pleading with us not to call 999..."it's just so embarrassing " he said.... He sounded like he was in his 50s, well-spoken and very fed up with himself! I stayed talking to him while James ran to get a longer rope and torch from home (but also gave me the nod to call 999). I was worried that our friend might let go or try to climb out and lose his grip if he knew the lifeguard was coming so I kept him talking and asked if there was anyone else I could ring for him. He rather ruefully said no. James appeared back sprinting with the rope. But seconds after speaking to 999, the Chiswick Lifeboat appeared heroically through the dark. They scooped our water-logged Bradley wiggins up and took him down to Kew and off to hospital to get checked over. We walked his bike back to our house and hope to reunite them soon. The moral to the story is that you should never flop on the sofa after dinner when you could walk round the block. Because that article was right - fart walks do save lives! Great job Chiswick Lifeboatshow more

NEW: Three men claim Sean "Diddy" Combs r*ped them... while they were unconscious between 2019 & 2022. The lawsuits were filed anonymously in New York, with all three men claiming they would be given drinks and would wake up to Diddy r*ping them. The following is disturbing: One man says he had met up with Combs to discuss pay he was owed. Combs served him a drink and in "two minutes, I start to feel like, just very tired, very sleepy." The man woke up with Combs r*ping him. When he told Combs to stop, the man was told: "I'm almost done." Another man, tells a similar story, saying he was given a drink at the Park Hyatt hotel for an afterparty. The man says his vision began spinning and he lost consciousness. When he woke up in between blacking out, he says he saw "a man and a woman sitting on the bed recording the r*pe on a camera." He says he was given $2,500 by the man who filmed the alleged r*pe the next morning. The third man says he attended a party at Combs' mansion in East Hampton, New York. He was given a drink and fell in and out of consciousness, according to the lawsuit. The individual says he woke up to Combs and his associates r*ping him. Source: NBC Newsshow more

Instead, he was executed by the regime in Iran, because he dared to ask for freedom. The silence around it is impossible to ignore. During the Russian attack on Kyiv on July 10,... Ukrainian surgeon Borys Todurov and his team performed a child heart transplant. Doctors transported the donor organ for a child patient at the Heart Institute from Okhmatdyt Hospital during an air raid alert, missiles and drones attacking Kyiv as explosions sounded in the city. On their way, the doctors recorded a video of burning buildings not far from the clinic. Despite the threat, the heart was delivered, and the transplant was performed. The patient, a child, has been living with a heart disease for several years, but her condition deteriorated, and a new heart was her only chance. Thankfully, the operation was successful, Dr. Todurov said. "The heart is working, and the pressure is stable. We hope that the patient will recover and live a long and full life," he said. Ukrainians are incredible. To perform such complex operations under explosions is just something out of this world. 📹 & 📷: Violetta Kirtoka/Facebookshow more

Humble day for a cardiologist… A patient of 44... years came with an axillary chest pain and scapular pain. (I have indicated the pain sites in the accompanying picture). He has had diabetes for the past three years and also has a family history of heart disease. He is not a smoker and doesn’t have any other coronary risk factors. His ECG, ECHO and TMT were normal. Because the pain was atypical and non-exertional and his non-invasive testing was normal, I prescribed medication and sent him home. He came back again with more severe pain at the same site after a month, and again, the ECG and Troponin were normal. I reassured him once more. He came back the next day (last Saturday) with the same complaints, but this time I suggested doing an angiogram to investigate the persistent pain. What I found was a 90% lesion in the PLV artery. I stented the vessel, and he experienced total relief from his chest pain. Physicians and cardiologists should be willing to modify their diagnosis when the initial line of management does not provide clinical benefit to the patient. Ego has no role in medicine. #chestpain #heartattack #medtwittershow more

This came out of nowhere. Took him to the... Vet ER last Sunday night. We were told he had double pneumonia and needed to stay. Yesterday he had a little bit of improvement. Early this morning the Vet called and said hurry to get here. When we arrived they were doing CPR so we could tell him goodbye. We literally had seconds and then he was gone. I’m heartbroken over this. Bugsy Malone celebrated his 10th birthday last month and he was a Seizure Alert dog for my wife who suffers from seizures due to having 9 brain tumors. Our daughter is having a hard time with this as she starts school this week. I appreciate those who sent us well wishes and prayers during this time.show more

With great regret, I come to inform you that... I am out of the next #UFC317 competition. I suffered a foot fracture during training and, after medical evaluation, I was instructed to have surgery and focus 100% on recovery. It's not easy to stay out of something I've been preparing for with so much dedication, and without a doubt this would be the most advantageous fight of my career but I understand that this is just another challenge on the way and I trust the plans that Allah has for me! I sincerely thank everyone who is with me - we remain firm, stronger with each obstacle. The battle was only postponed... I'll be back soon!show more

I told my 65 year old father NOT TO... GO to the Pudgy Pop up! My parents live in NYC, I asked them about the pop-up to see how close it is. Turns out it's 50 minutes and 2 train rides away. I'm not gonna put him through that journey. Saturday night, he calls me up & sends me this video In the video, he says: "This is very very very incredible. So very! It's very beautiful!" I share this to say this is a 65 Yr old man who knows nothing about this world except what I share with him. He went to casually pick up an item & surprise us, waited in line, & got the warmest treatment from the team. Walked out with the Pudgy Bouquet for us and the Pax & Polly keychains for him and my mom (jealous) and stories he could not stop talking about The phone call was literally him just going on and on about the vibe, the staff, the smiles, the people and everything in between. My dad went to his first Pudgy event before I did & I'm happy for it! Thank you Pudgy Penguins for putting a smile on my old man's face and getting him to feel the Pudgy experience IRL. 🚨 Michael Kayode after receiving Arda Güler’s No. 10... jersey: 🗣️ “Honestly, I was incredibly happy. It was a childhood dream for me, and I’m sure my wife will be so proud of me. Arda Güler is a special player. Even though we won 4–1, he completely dominated the midfield and showed just how talented he is. Watching him play up close was something I’ll never forget. I’ve followed what he has achieved with Türkiye and Real Madrid, and honestly, he is one of the most gifted midfielders I’ve seen in a very long time. The way he controls the game, his confidence, his intelligence and the quality he has at such a young age are incredible. I truly believe he is destined for something very special, and he will go very far in this game. As for the jersey, I’ll wash it, keep it safe and treasure it until my children are grown enough to understand what it means. This is more than just a jersey for me. It’s a memory I will cherish for the rest of my life. It is a real honour.”show more

🚨🔴 Marcus Rashford MBE! 💰 350k per week -... £18m per year! 🧢 37 Appearances in all competitions ⚽️ 8 Goals 🎯 6 Assists He was never a good pressing player and he was never good on the defensive side, he was always a liability on our squad when the opposition attacked us! But he was performing on the attack and somehow he was able to make for his lack of defensive effort, but with this stats of this season.. and him jogging on the field just like he is in training is the reason why i think is for the best for both parties to move on from each other. I know he is a local boy, and he loves the club… but man he needs to wake up or he needs to get away from our club, us as fans can’t defend him anymore, we can’t get behind him with this kind of performance, we see no desire no passion nothing!!! That’s shocking for a player that is entering his ‘’prime’’ Dear MR wake up, or please get out of our beloved club! #MUFCshow more
