Ketogenic diets in the ICU? The research is beginning to explore it. In a randomized controlled trial of 40 patients with sepsis, investigators tested a medically supervised ketogenic diet during critical illness. All patients in the ketogenic group achieved stable nutritional ketosis, with no serious adverse events attributed to the intervention. After day four, none of the ketogenic patients required insulin — compared to 35–60% of patients in the control group on any given day. The ketogenic group also experienced: • More ventilator-free days • More vasopressor-free days • More dialysis-free days • More ICU-free days Researchers observed signals consistent with reduced immune dysregulation and lower inflammatory signaling — a meaningful finding in a condition defined by metabolic chaos and systemic inflammation. This was a small, early-phase trial. It does not establish standard of care. But it adds to a growing body of evidence that metabolic therapy may have applications beyond chronic disease — including critical care. Breakthrough Update: Today, Vertex announced that they are beginning their Phase 3, or pivotal trial for VX-880, a stem cell-derived islet therapy. A pivotal trial gathers the data required for an FDA submission. This therapy is for people with type 1 diabetes (T1D) and hypoglycemia unawareness and severe hypoglycemic episodes. This is a HUGE first for the #T1D community as it is the first time a scalable cure for some people with T1D is entering this phase. Today we’re reporting interim Phase 1 clinical data for our REC-617 monotherapy trial – with plans to expand into combination studies in advanced solid tumors. At the AACR Special Conference in Cancer Research, CSO David Hallett shared interim monotherapy dose-escalation data from the Phase 1/2 study (ELUCIDATE) of REC-617, a selective CDK7 inhibitor, in advanced solid tumors. 🔹The interim Phase 1 clinical data for REC-617 included: ▫️Dose-linear pharmacokinetics (PK) with rapid absorption and robust pharmacodynamic (PD) biomarker modulation, suggesting substantial target engagement; ▫️Confirmed partial response (PR) during monotherapy dose-escalation in a patient with platinum-resistant ovarian cancer, treated with 4 lines of prior therapy in an advanced setting, with durable response ongoing after more than 6 months of treatment; ▫️In 4 additional patients, a best response of stable disease (SD) for up to 6 months of treatment. Dr. Hallett noted: “Cell cycle dysregulation and transcriptional 'addiction' are both hallmarks of many aggressive cancers. 🔹 AI drug discovery, validated. Recursion recently announced the first clinical validation of its AI-enabled drug discovery platform – positive Phase 1b/2 data for the ongoing TUPELO study evaluating its AI-identified drug, REC-4881, in Familial Adenomatous Polyposis (FAP). This progressive rare disease affects more than 50,000 people in the U.S. and EU and has no FDA-approved treatments. It results in hundreds to thousands of polyps developing in the colon, and requires a lifetime of invasive surveillance and surgery. Recursion recently announced the first clinical validation of its AI-enabled drug discovery platform – positive Phase 1b/2 data for the ongoing TUPELO study evaluating its AI-identified drug, REC-4881, in Familial Adenomatous Polyposis (FAP). This progressive rare disease affects more than 50,000 people in the U.S. and EU and has no FDA-approved treatments. It results in hundreds to thousands of polyps developing in the colon, and requires a lifetime of invasive surveillance and surgery. Recursion used AI not only to deliver the first potential treatment for this disease but also to quantify – for the first time at scale – the true natural progression of polyp burden in the trial-relevant population.
