In a randomized controlled trial of 40 patients with sepsis, investigators tested a medically supervised ketogenic diet during critical illness. All patients in the ketogenic group achieved stable nutritional ketosis, with no serious adverse events attributed to the intervention. After day four, none of the ketogenic patients required insulin — compared to 35–60% of patients in the control group on any given day. The ketogenic group also experienced: • More ventilator-free days • More vasopressor-free days • More dialysis-free days • More ICU-free days Researchers observed signals consistent with reduced immune dysregulation and lower inflammatory signaling — a meaningful finding in a condition defined by metabolic chaos and systemic inflammation. This was a small, early-phase trial. It does not establish standard of care. Ketogenic Diets Change Microbiome to Help Multiple Sclerosis (90 sec Breakdown of New Data) Background: Ketogenic diets have been shown in human clinical trials to improve multiple sclerosis. But HOW? 📝 A new mechanistic paper reveals ketogenic diets cause intestinal cells to make ketones, which shifts the microbiome to lead to production of another metabolite - indole lactate - that inhibits inflammatory cells ❌🔥 🤓 Nerd Note 1: “intestinal” production of ketones was important such that when the ability to make ketones locally in the intestines was knocked out (but liver ketogenesis remained intact such that circulating ketone levels were not impacted) the neuroprotective effects of the ketogenic diet were diminished 🤓 Nerd Note 2: Exogenous ketones also worked in this animal model, suggesting it is the ketones that are having the effect (here changing the microbiome to confer neuroprotective properties) Of course, more research is needed but this is exciting and promising.
