The game will make foliage constantly change lods and pop-up. There is no way you shot this on vanilla. What mods/tools have you used?

Just vanilla. Using @FransBouma's tools to escape the limitations of the photomode.

May I ask how many attempts you did to make Geralt correctly walk that path from that angle

How you do this? I mean the camera not move, unless it's mod? It's so cool I loved panoramic video like this, I hope I can make something like this

I use @FransBouma's camera mod on PC. I set the camera, and then just walk into the scene.
